  6. at each of the update points.  Revisions left by other means
  7. will not be incorporated into the list.  This assures that
  8. the list contains complete, accurate information.

  10. When leaving information, be sure to COMPLETELY fill out the
  11. questionnaire even if you are only changing certain
  12. information. This allows the editor to verify current
  13. information.

  64. locations.  ("xxx" is the version number.)

  67. the month for the next month unless the first falls on a
  68. weekend.  New versions are usually available sometime Sunday.
